Registered Nurse

 

We are looking for a Registered Nurse (RN) to coordinate and administer exceptional, compassionate home care to our clients in the Leominster area.

 

Summary:

The RN is responsible for evaluating prospective clients and determining their eligibility; developing detailed client Care Plans; managing Care Aides; providing skilled, in-home medical care; and coordinating visits from other care providers as well as hospital admissions.

Reports to:

Agency Manager/Administrator

Responsibilities:
  • Assess each client’s home environment and needs and determine eligibility
  • Develop detailed, complete Care Plans and initiate Nursing or Aide’s Care Plan and/or Homemaker/Companion Service Plan
  • Coordinate client care, making sure Care Plans are properly implemented
  • Lead care team and supervise Care Aides in the home; attend/lead staff meetings
  • Assist with training of Care Aides, including in-service training programs
  • Arrange in-home visits and hospital admissions; make referrals to other agencies or services as needed
  • Provide services based on physician’s orders, such as urinary catheter insertion and care, venipuncture and administration of IV fluids or medications, and dressing changes
  • Administer medications and monitor clients’ progress
  • Teach, counsel and instruct clients and their families in appropriate home care knowledge and skills
  • Regularly evaluate the effects of care provided and assess client’s needs, revising Care Plan as necessary
  • Maintain clinical records, prepare clinical and progress notes on specialized nursing services, and report any changes in condition or deviations from Care Plan to the client’s physician
  • Complete paperwork associated with all Care Plans in accordance with Agency policies and Federal/State law
  • Interact with other disciplines as needed
Qualifications/Prerequisites:
  • Degree from an accredited school of professional nursing, and current RN license through the State Board of Nursing
  • Minimum one year’s nursing experience, preferably including home care
  • Strong physical stamina and mental health, as demonstrated by pre-employment health clearance
  • Driver’s license and clear driving record
  • A vehicle in good working order and insured in accordance with state and/or Agency requirements
  • Excellent writing and charting skills
  • Ability to pass a background check and drug screen
